TM6 Arrival Form is a form issued by the Thailand Government According to the Immigration act B.E 2522. It is necessary to be filled by the Non-Thai passengers while they arrive at any international airports and Land borders in Thailand. This form is an essential requirement, so you can not pass the immigration if the TM6 form is not filled correctly.

TM-6 is one form, but there are two parts of it, and there are fields on both sides of the TM6 form that has to be filled.

Arrival Card has the following fields to be filled:
Family Name (Last Name)
First Name
Middle Name
Gender
Nationality
Passport number
Date of birth
Flight Number (Write the Code Number of Flight You Arrived from, for example, TG333)
Visa No (If you already have the visa then write its Number, if you belong to visa-free countries then leave it empty)
Occupation
Country Where you Boarded
Purpose of Visit
Residence City
Residence Country
Address in Thailand (Write the Hotel name and its address / write the Address of your invitee / Your Airbnb Address)
Telephone Number (Write your telephone number)
Email (write your email address)

Departure Card
Its more important than Arrival – Thailand immigration will return this section of the form after stamping on it.
You have to show this card to Thai immigration at the time of departure from Thailand, so don’t lose this section, and you must staple it with your passport.
The departure card has this information to be filled:
Family Name (Last Name)
First Name and Middle Name
Date of Birth
Passport Number
Nationality
Flight Number (its a Departure Flight Number or Flight Code of your Return Flight, for example, TG332)

Back Side of TM6 Form

The Back Side of the TM6 form is also a part of the Arrival Portion, and it has a little tricky question, which is “Port of Disembarkation,” and I have explained it in a very easy way.
Below are the all the fields on the back side of TM-6 Arrival Form.
- Type of Flight – Charter – Scheduled
- If you are using a commercial flight, then that should be “Scheduled” Flight

What is the Port of Embarkation in TM6 Arrival Card?
Port of Disembarkation means the city or the place of your return flight from Thailand, So if your return flight is from Bangkok, then Write “Bangkok.” If its from Phuket, then write Phuket as the Port of Disembarkation.
